Hey guys,

It's been a while since I had to ask a Question here, but the time has come for my wife and I to remove the conditions on her Permanent Resident card and we would like to also change the last name on her card.

Currently her Permanent resident card shows her maiden name, but her Social Security card is in her married name.

And because of this simple error the DMV in Florida refused for the past 2 years to allow her to get a license.

I called the USCIS three times already and received the vaguest, most eh, whatever, responses from the operators and I'd really like to avoid any Request for Further Anything. Basically I was told to just fill out the I-751 with the married name... that's it! But after years of dealing with visa paperwork where a simple miss-placed "n/a" causes questions, I seriously doubt anything's so easy.

Has anyone else on this forum done something similar and can shed some light on it for me?

Is there a form or what is the best way to go about informing the USCIS when we are filling form I-751 that we would like the new card in her married name?

Would you not just fill out all the forms in married name? then include a copy of the marriage certificate the same as you would with AOS?

Hi,

My first green was on my maiden name, SSS card on married name.

When I filed for I-751, on Part 1 Information About You, I wrote my husband's surname as Family Name/Last Name; my maiden name/surname as Full Middle Name

(that is how I want my name on the new card.

On Part 3, Additional Information About You, Other Names Used, I wrote my full maiden name as written on the old green card.

I did not send copy of marriage contract, as they were not required.
