I have a problem, i live here in central oregon its a small town and the doctors will not sign my I-693 if i will not have a medical exam...I told them i just need a I-693-A supplement the TD shot. and they said i need a new medical exam because my medical exam b4 was perform in philippines and it is not granted inside the US.. this really maked me mad they dont listen..pls help me what to do...

Sorry to hear you are having issues with the civil surgeon.... all you can do if phone other civil surgeons on the list and find one who will do what is needed... even if it means you have to travel....

You should have posted this in the Adjustment of Status forum as you are at least 3+ years away from citizenship...

Hello neighbor,

You don't say exactly where you are. If there are a choice of doctors, you can go somewhere else. If there is no other doctor nearby, you can try to argue with them (see the following links). If arguing doesn't work, you can pay for a the exam they offer to get what you need. Or, you can go to any other civil surgeon in Oregon; there are several ifyou don't mind making a day trip out of it--you can go to Hood River or East to Burns.

Are Medical Examinations (Form I-693) and Affidavits of Support (Form I-864) required at the time of filing the I-485 package?

Yes, the Affidavit of Support (if required) and complete Medical Examination should be filed with the I-485 package. The NBC will send a Request for Evidence if either of these Forms is not included with the initial I-485 filing, which will delay the processing of your case. The I-864 and I-693 Forms should include any supporting evidence requested by the instructions.

All applicants required to submit Form I-693 Medical Examination are also required to submit a Supplemental Form to I-693 (immunization record). Applicants who last entered the United States as a K nonimmigrant (I-94 Arrival/Departure Record and/or non-immigrant visa page in passport shows K-1, K-2, K-3, or K-4 as admission code) are only required to submit the Supplemental Form to I-693 (immunization record) with Form I-485. K nonimmigrants are not required to submit Form I-693 (medical examination).

If you are not satisfied with the above, you could consider contacting the District Director for USCIS in Portland.
