we got the interview date on feb. 14 and are trying to prepare right now for husbands medical exam we read the info on the consulate website and it says we need several forms DS-2053, DS-3024, DS-3025 and DS-3026. It mentions that the NVC gives out these forms but when and if they do? We only were emailed the interview letter from NVC and the interview prep info on the NVC website doesn't list these forms so how do we obtain current versions of them?

Congrats on the Valentine's Interview Date!! :)

In my case, I only needed to bring the interview letter, passport, and 2 pictures. However I searched on google and those forms are basically all medical forms (check it out here: http://photos.state.gov/libraries/vietnam/8621/pdf-forms/DS-2053.pdf )

I did see that form briefly while I was at the doctor's office, but they are the ones who had it and filled it out. Perhaps you might want to call the doctor's office and inquire about it, or e-mail the consulate directly.

DS-2053 is Medical Examination for Immigrant or Refugee Applicant

DS-3024 is Chest X-Ray and Classification Worksheet

DS-3025 is Vaccination Documentation Worksheet

DS-3026 is Medical History of Physical Examination Worksheet
