How to go about getting the Immigration Physical?

I got the list from the USCIS website, and I found a doctor and his fees are $240. Apparently they need my vaccination records and all other ORIGINAL records from my doctor in Canada. I call up and apparently I need to fill out a Medical Release Form. Now, where do I get one of these? From Canada, or from the US? Do I let my new doctor's office get them? I thought it was my responsibility? I'm just really stressed as I thought this would be one of the easier things; go in, pay up, leave. I'm only legal here till February 18, as a Canadian can only be here for 6 months at a time (I got here August 18 2010 visiting). I married my husband on December 28 2010, and all the forms are filled out. It's just this that is holding us up. How long will it take for them to send down the forms, my new doctor to go over them, and then call me in for a physical? The last thing I need is to be here illegally. Someone please help?

You will need to go to one of the 4 Panel Physicians in Canada for your medical, since you will be dealing with a consulate in Canada. It can't be done in the US. There are 2 in Montreal, 1 in Toronto and 1 in Vancouver (if you look through the Canada fourm you will find thread with reviews for all of them). Many people wait until they have their interview scheduled, then call and make their medical appt. You need about 2 days before you get your results so plan to head to Montreal a few days in advance of your appt.

As for your vaccination records, if you do not have them, go to your family doctor and as to have titre tests done for the vaccinations that you require. You will get a report from the lab stating your immunity levels...if you are not showing immunity, then you haven't had that vaccination and will need to get that.

It depends a lot on what exactly you are applying for. If you are in the process of applying for a spousal visa, then yes, you will need to do your medical when you return to Canada, which you will eventually have to do for the visa interview in Montreal anyways.

If you are simply adjusting status from being a Canadian tourist, you stay in the US and get your medical at one of the many Civil Surgeons listed on the USCIS website. If the CS is telling you that you need immunization records from Canada, and the Canadian doctor is being a pain, do what colleens said above: get titre tests for the vaccines you can, and just redo the rest. I remember my Canadian doctor was very reluctant to write any kind of letter or release any kind of records for the purposes of immigration. Just sidestep the Canadian doctor as much as you can.

As for transferring medical records... I signed a release form at my New doctors office.

I just re-read the post and yes, it does seem that way. In that case OP you can go to any of the Civil Surgeons listed on the USCIS website. If there is more than one in your area, call around as they are all different (anothe CS may not require your medical records). They also charge different fees so you may find one that is cheaper as well.

I would still see about getting your titre tests done (they may be able to get the results sooner than a month) to avoid having to get any already administered vaccines. Some of them can be pretty costly.

Worse comes to worse, I guess I can get the vaccinations over with, and try to talk to them at the appointment about everything else (though they said that they want everything first before we have an appointment). Ugh, out of the millions of people in Canada, I have to fall in love with one from another country. Don't get me wrong, I love him dearly; been together for almost 5 years (!) but damn it's hard!

I didn't know you could do that, about finding out what vaccinations I've already received. I'll have to call them and see how much they charge for it, as it would make everything that much better. Did you have the same family doctor for a long time? If so you could call them and see if they send you out a note stating what you medical history has been like and what it was the last time they saw you.

I hope you can get the medical part cleared up easy and get on your way with filing.

Not sure I can really help - Most of my medical records from my schools my mom had copies, my doctor wrote up one of thos orangy-yellow books with shots and dates and signed all of them. I used thos for my medical in canada and brought them with me to the states, the civil surgeon said the orangy book was handy and liked that lol

Tri - I think you're in a different position then the OP? You're still in Canada and going for your medical in Canada - correct? I don't think this is an issue for you.

I had a note like this.. My doctor sent it to me through the mail.

I agree - call your former Cdn doctor and find out whether they can release medical records to the immigration medical doctor or to you. They may be able to do something for you.

If not - do you have a new family doctor here in the US? They may be able to contact your former Cdn doctor with your written permission to get those records or they could provide a current history of your last 4-5 months here. Obviously there are ways around not being able to get the medical history or immunization records - like Marilyn mentioned - go to your own family doctor here and have a titre test done to establish was you've had before. Get any other immunizations updated by them now so you have proof now.

Luckily my mom still had my immunization card. But All I had to do was call the Regional Municipality of Halton (Local for me) and they sent me one. I received it 4 days later. Or do they not have your immunization charts after a certain amount of time?

I got a immunation card sent to me too from the Durham health department... the local health department area that you went to school in. Even if they only have till you became an adult, it might help :)
