Hello everyone :)

My fiancee from Indonesia has a question with regards to the "family name." She told me that most Indonesians do not have a family name. My fiancee has got a first and middle name only.

In that case, what should she write in the "family name" box?

2003 - met online

12-09-2010 - met for the first time

12-11-2010 - engaged

06-07-2011 - married

09-27-2011 - I-130 sent

09-29-2011 - NOA1

04-04-2012 - I-130 approved

05-14-2012 - NVC Case number received

06-26-2012 - Case completed

08-02-2012 - VISA APPROVED :)

Mine also like hers. I put my first name on "FIRST NAME" box, and my middle name on "FAMILY NAME". For more accurate information, you better contact Jakarta USEM via email. They reply it quickly, i used to get the reply within 2 days. If they still not reply within the time, i resend the email and always get the answer soon. Better spend a little bit longer time to prepare your paper then dont feel safe once u send it off. Good luck on your journey.

My birth certificate didn't record a family name, but when I applied for my first passport they added my dad's last name as my family name.

I think it's very important to make sure that the name she uses on the paperwork are the same as her names on her IDs, passport, etc. So, yes, she can use her middle name as her last name.

The only document that stated my name without family name is birth certificate, but because it stated my dad's full name as the father.. I didn't have any problem.

For more accurate information, you better contact Jakarta USEM via email.

Okay I will email the Jakarta US Embassy. Could you please tell me what their email address is? For some reason I cannot find their email contact on their website. I'm only given a phone and fax number :help:

May I ask where you got that address of JakIV@state.gov ?

I got it from Jakarta USEM. Just forward your email to that one. Bookmark the Jakarta USEM on your computer. We gonna need many information from it: jakarta.usembassy.gov
