K1 Visa Waiver???

Timeline

I am in the process of filing for a K1 visa. I had a visa approved last year for another person, but the relationship terminated not too long after filing. I petition is expired. I am filing for a K1 visa for a different person. The instructions says that since I filed for a K1 visa within two years that was approved, I must apply for a waiver. It doesn't mention how to apply for a waiver or where to apply. Where do I find waiver forms for a K1 visa and where do I file these forms?

09/08/2008 - Began emailing each other

07/04/2010 - Began communicating regularly

12/25/2010 - Met in Australia for holidays

12/27/2010 - Became engaged in Margaret River

01/20/2011 - Purchased ticket to visit her in Colombia

01/29/2011 - I-129F Sent

01/31/2011 - I-129F received and signed by USCIS

02/09/2011 - Check cleared bank

02/11/2011 - Received NOA1

02/14/2011 - Touched

02/17/2011 - Service Request response received

03/02/2011 - I-129F Approved

03/08/2011 - Received NOA2 approval letter

03/21/2011 - Case forwarded to Bogota

03/24/2011 - Received NVC letter. Sent email to Bogota Embassy.

03/25/2011 - Received response from embassy to download instruction package.

03/30/2011 - Emailed fiancee DS-230 & DS-2100 to embassy for interview.

03/31/2011 - Received confirmation of documents submitted to embassy.

05/10/2011 - Visa interview scheduled but later postponed by us.

07/19/2011 - Visa Approved

07/30/2011 - Visa picked up at Domesa office.

no form... you simply write on a piece of paper the previous beneficiary info, facts and particulars as to what happened and close with I hereby request a waiver of the multiple filing limitation.

When the relation terminated, I assumed that the petition would expire after the grace period. According to the approval notice the petition would have expired on 08/18/2010. I have also read in the K1 documentation that the application expires one year after it is filed, so I didn't think that I would have to communicate with any agency of my desire to not continue with the process.

When I began looking into filing for my new fiancée, I am thinking it is a good idea to ensure that the process is truly canceled. I thought about either calling the embassy in Bogota or the USCIS Service Center or sending them a letter asking that the previous process be canceled. I guess my dilemma is which should I communicate with? My case was already sent to the Bogota embassy back in May 2010. I also thought about just including this letter asking for cancellation of the previous process in the new application along with a request for a waiver.

I would appreciate any suggestions as what I should do and which route to take???
