I currently have a K1 Visa and I am in the process of starting my application for Adjustment of Status. I am a bit confused with respect to the form I-693 as it is saying if I already had a medical I don't have to have another one as it is under a year since my last exam. However, it states that although I am exempt from the medical exam (as I had one in order to qualify for K1 Visa) it states that it is mandatory to get a civil surgeon to fill out this form (I-693) to verify vaccinations. I just want to ask if this is infact true because I have seen other conflicting information saying I don't need to fill out this form for adjustment of status. If anyone could verify this who has gone through the AOS process would love to hear back. Also, if anyone has had to go to a civil surgeon does anyone know how much it costs for this vaccination verification in the US?

Typically a K1 does not need to complete the i693 like you said as long as you are filing within a year on the foreign medical exam. I have completed the AOS process (interviewed yesterday) and was not required or requested the I693 form (but I did recently second guess myself on this too just before our interview!). In my AOS application though I did include a copy of the vaccination worksheet I received with my sealed medical from the clinic that did my medical. Also search the forum for "AOS form i693" you'll get lots of info that way:)

I also recently submitted for AOS from a K1 (from Canada) and just included the Vaccination Worksheet from my medical. I'm hoping for no RFE because of this, but if it happens, I guess it's not the end of the world. I stressed for a day or two before coming to this decision. It looks like the time to see the CS, get the appointment done, and then wait for them to get the results of whatever they want to do, and then fill out the form would have taken at least a week.

I agree that the wording is very ambiguous, and it seems that the process is confusing to everyone, even civil surgeons who are not familiar with every type of visa out there.

If your vaccinations were checked off as "complete" or "eligible for blanket waiver," you can consider just including the Vaccination Worksheet. If your vaccinations are checked off as "incomplete," then I believe you need to see a civil surgeon, and get vaccines updated and documented on the I-693.

I called about 6 or 7 civil surgeon offices in my area, spoke to the secretaries, and also a couple civil surgeons over the phone, who tried to charge me anywhere between $150 to $350 to "get the paperwork done," or do a whole physical all over again (no thank you). I have seen some threads where people are posting lists of CS who will just do the transcription for $50 or less.

Thanks for your response. The only thing is that I had to submit my sealed medical package to my K1 Visa Interview and they never gave it back to me so all I have is the vaccination worksheet. Do you know if this is okay? Let me know.

We all had to submit our sealed medical package at the K1 interview and it becomes part of your USCIS file. And yes I also have the vaccination form copy and mine is marked complete (also @lovetimhortons said his was) it seems people have had fewer issues submitting a complete one vs one that identifies you as missing vaccinations. And of course others have decided to do the I-693 from the get-go or after an RFE.

Personally if my vaccination worksheet said complete I would submit that and be done with it. If there are vaccinations you are missing you might be best suited to go to a civil surgeon and get that sorted out first...its up to you :)

Also as I may have said before the vaccination worksheet and lack of I-693 was not brought up at my AOS interview either. For K1 our medical exam is already complete (unless you are missing vaccinations).

Personally, my vaccination worksheet was not labeled complete, and I was still approved without the I-693. This is back with they still required the HPV shots, and I started the series right before the medical but didn't have the time to finish it.

It seems like USCIS has stopped cracking down on the I-693 "requirement". There was a time in 2009 and 2010 they were sending out tons of RFE's for them, but I haven't seen many lately. Of course, they still happen, and USCIS still occasionally loses medicals, which is rare, but happens. If they lose your medical, you need to have a new medical done. But don't worry about that! That's something you can't control.

csmith: yes, I submitted just the "Vaccination Worksheet" from my medical in Canada.

according to many recent Canadian VJers doing AOS from a K1, most seem to have AOS'ed with just this "Vaccination Worksheet," and without the I-693 filled out. within the sealed envelope is supposed to contain the official "results" of your medical examination.

the wording on the AOS instructions, and the I-693 instructions is pretty ambiguous, if you ask me!! further complicating this issue is the whole DS-3025 thing (which is identical to the "Vaccination Worksheet" we receive in Canada, but has DS-3025 written on it. it seems people in the UK and Australia receive the actual DS-3025).

if you do decide to do the I-693 transcription, just remember you only require certain sections filled out by the CS, and to take the AOS & I-693 instructions with you, as the secretary/CS may or may not be familiar with a K1. also take any vaccination records you have from Canada, as well as anything you received from your medical in Canada. and don't forget to take a big wad of cash to pay for the transcription:) (jk)!!
