How does my Wife go about getting permission to leave Kazakhstan, from who, and when should she start the process?

Exit visa are remnants from the Sovjet era. Before citizens were allowed to permanently emigrate to another country they had to obtain an exit visa from the OVIR (immigration police).

According to this publication of Human Rights Watch, Kazakhstan doesn't require exit visas anymore since 2002. My own experience is that such news is only slowely propagating to the local officials at the OVIR, often because they use it as a reason to ask for bribes.

Please keep in mind that neither Human Rights Watch, nor Go Kazakhstan! are authorative sources for Kazakhstan laws, so to be sure you should contact either the OVIR or a lawyer in Kazakhstan.
