Hi!

The stress starts again...

I just set the appointment with the Civil Surgeon, it will be next wednesday and I have two questions:

1.- The Notice of Action for the Initial Interview ask us for the I-693 medical examination, but somebody say something about the I-693A form (inmunization record), I printed out both forms but which one the Doctor is going to use. :unsure:

2.- I had a medical examination last april 3rd/2006 as part of the k1 process (we crossed the border the next day when the k1 was approved), so Do I need another medical examination because de I-693 format? :angry:

3.- The Doctor's secretary told me about $170 or $260 if I need shoots and $70 for my daughter, is that expensive? :o

2. You don't need the full medical again if you did it for K-1. You just need to get the I-693A filled in. This should be cheap.

3. That's cheaper than I paid. $250 for the medical, plus extra for the shots. I didn't have much choice however, I only had 1 civil surgeon within 100 miles of me.

You don't need another medical as long as your K-1 medical was less than one year from the date you send in your AOS application.

We paid $35.00 at a civil surgeon to have the Vaccination Supplement filled in (no vaccinations were required).

the civil surgeon has the form so you don't need to bring one with you.

if you are a k-1 and your medical is less than a year old, then you don't need to do another one.

for AOS, you must have up to date vaccinations (varicella aka chicken pox, measles/mumps/rubella, and diptheria/tetanus). and the hepatitis B series if any of you are under 19 and influenza shot if you are over 65 (i think its 65, i won't swear on it :blush: ). someone had the guidelines link somewhere for this but i can't find it just now... sorry.

if you already had chicken pox then you won't need a shot. if you are unsure, then you can either get the shot or get a blood test (titer) to check for immunity. you can ask the doctor how much the titers would cost. i think its more expensive though... :unsure:

if all of your vaccinations are complete, then you only need to carry the records and/or the copy of the DS-3025 vaccination worksheet (that you should have gotten from your panel physician at home) and let them transfer the info to the I-693A. if you didn't get the DS-3025 then call the panel physician and have them send over a copy.

the civil surgeon charged us $35 to fill out the form. i was missing a vaccination so i got it done at my doctor's office in PA. it was covered under our insurance so it was $10. i am not sure how much the shot would have been otherwise... probably quite a bit.
