I am Really Thankful for the various forums and forms posted on here, it has really helped us in preparing our forms. I am a current F-1 student (In USA), married to my US citizen wife, and planning on applying for I-130, I-485, I-765 and I-131 simultaneously.

However, I do have a small problem/doubt as to "How to Correctly" place my name in these forms. I only have "2 words" in my name, unlike most people who have 3 words. For Privacy reasons (no offense intended to anyone) lets say, the "first word" of my name (Which is my First name) is ABCDE and "second word" of my name is WXYZ.

I have written my name, ABCDE WXYZ (consistently) as follows: Last name (WXYZ); First name (ABCDE); Middle name (N/A)

in the G325A, I-130, I-485, I-864, I-765, I-131and I-693 forms for my USCIS application. Please kindly advice as to the appropriate location of “ABCDE” and “WXYZ” for these forms having read the information below.

Following are the “variations” of my name in these documents:-

1) Passport:

Surname: - (its a dash, and not left blank)

Given Names: ABCDE WXYZ

2) Current US visa (F-1), issued in 2009, used for my last entry to USA:

Surname: ABCDE WXYZ

Given Name: FNU (which I think stands for First Name Unknown)

3) Old US visa (F-1), issued in 2003, used for my First entry to USA (which has been crossed and "CWOP" written on it by an immigration officer upon my last entry):

Surname: WXVZ

Given name: ABCDE

4) I-94 card:

Family name: (Its left blank)

First (Given name): ABCDE WXYZ

5) Current and all previous I-20s (except #6 below):

Family name: WXYZ

First name: ABCDE

6) First I-20 used at my first initial entry to USA in 2003, along with my #4 old Visa (F-1):

Family name: LNU (which I think stands for Last Name Unknown)

First name: ABCDE WXYZ

7) Ohio driver’s license:

1 WXYZ (Line one of license)

2 ABCDE (Line two of license)

So is it correct if I put my name ABCDE WXYZ as: Last name (WXYZ); First name (ABCDE); Middle name (N/A) for ALL these forms or how do I put my name?

Do I need to write a Statement (If so, what do I write there?) on a separate sheet of paper, explaining the name issue and cite the # 1 to # 7; and include that in my package for USCIS?

** moved from "Adjustment of Status (Green Card) from Family Based Visas" to "Adjustment of Status from Work, Student, & Tourist Visas" as you are adjusting from a F1 visa and the prior forum is for K1, K2, K3 & K4 **

Go by ur statment on number 7, then u may need to explain to the adjudicator -IO- of whats going on with ur name, which can cause a new background check to be run delaying ur approval, you can always make an infopass appointment and ask USCIS directly, which i think its best in your case, sorry about my previous post.

Raffy thanks for your input as to go with the # 7 License answer and also for your infopass appointment advice:)

Raffy, I forgot to say that my birth Certificate (as per your first reply) only says :Name of Child: ABCDE WXYZ. There is no First name, Middle name, Last Name etc columns/places on my birth certificate.
