I am applying the following terminology:

"Authorization Letter", in my case an email message telling me I could schedule.

"Confirmation Letter", the one that confirms the appointment date for the interview.

What is packet 3 - I don't know.

Will USEM send it - I don't know

I-134 notarized - since you sign it under penalty of perjury, it does not have to be notarized, or so I am told

Financial documents supplied by others, I am having them notarize them, I do not know if you have to.

Original NOA2 for medical - I hope a copy is good, that is all that I have.

Yes.

Packet 3 is the email/snail mail the USEM sends you, telling you that you are eligible to schedule an interview appointment.

should we have our I-134 & employment letter notarized?

Nope. As a matter of fact, if you are using an I-134 which has space at the end for a notarization, then you are using the outdated form. The employment letter is not required to be notarized. However, it is a good idea if it is printed on company letterhead.

its asking if the petitioner Intend or Do not Intend to make specific contributionsto suppor the person in item #3

You will get an Email after scheduling the interview. We used this for my wife to get her medical and entry to the embassy. A letter was sent to her as well but arrived 2 weeks after her interview!

what is packet 3? I received a letter referring us to the USEM website where procedures and forms are available for download and filling out. We received no "packet". Just a guess but this "packet" may refer to the older system USEM used prior to the web.

should we have our I-134 & employment letter notarized? Use the NEWEST I-134 form online and it does not require a notary. An older I-134 form is still valid and it DOES need a notary. Use the I-134 without the notary box at the bottom.

My employment letter did not require a notary (as of july 09).

do i need to have the ORIGINAL NOA2 for medical/interview or a scanned copy id fine? I sent a scanned copy to my wife who then just printed it out in an internet cafe, she had no issues at St Lukes or USEM. Just make sure you have ID. My wife used postal id and her school id.
