Hello All: Am working on the supporting documents for I-485. I recently relocated from Canada on a K1, and have married :)

Had a quick question that I was really hoping someone who recently filed for AOS (from a K1 from Canada) could answer!

Basically: What do I need to include with my I-485 to prove my vaccination history?? The instructions from I-485, as well as I-693 are unclear to me. The instructions have been cut and pasted below...

My vaccinations were all complete before my medical in Canada (2 months ago), and my medical was clear.

After my medical (in Toronto) I received a copy of a "Vaccination Documentation Medical Worksheet Two," which indicates that my vaccinations are up to date. It does not say "DS-3025" anywhere on the page. I called Dr Seiden's office in Toronto, and the secretary confirmed that it is NOT a DS-3025, and they cannot release the DS-3025 to me. She told me to go to a civil surgeon in my area and get the vaccines from that worksheet transcribed to a I-693. She also told me I'd likely require another medical. Arrrgh.

I've called several civil surgeons in my area, who seem to be under the impression that I need to undergo a complete immigration medical, and want to charge me anywhere between $115-$250 for the "paperwork." I'm happy to do this if necessary, but wonder if it is really necessary? No one seems willing to just fill out Section 2 (Vaccination Chart) and Section 5 of the I-693 for me. (Section I will obviously be completed by me). Regardless, I made an appt with a civil surgeon in the area whose secretary seemed pretty knowledgable, and told me they did another K1 I-693 last week.

As below, it seems strange to automatically assume that "the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report." Isn't that one of the entire points of the medical to begin with?

Help!!! (Thank you in advance for any answers).

From I-485 Instructions:

7. Medical examination

When required, submit a medical examination report on Form I-693, Report of Medical Examination and Vaccination Record.

Individuals applying for adjustment of status:

A. General: When filing your Form I-485, include your medical examination report with the application, unless you are a refugee.

B. Refugees: If you are applying for adjustment of status 1 year after you were admitted as a refugee, you only need to submit the vaccination portion of Form I-693 (pages 1,4, and 6) with your Form I-485, not the entire medical report, unless you had a Class A condition noted on your overseas medical exam.

8. Fiancé(e)s

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your Form I-485.

From I-693 Instructions:

2. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?

If you were admitted as a:

A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or

B. as a K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

C. You received a medical examination prior to admission, then:

a. You are not required to have another medical examination as long as your Form I-485, Application to Register Permanent Residence or Adjust Status, is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

b. Even if a new medical examination is not required, you still must show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original, overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit Part 1, Information About You, Part 2, the vaccination chart and Part 5, the Civil Surgeon's Certification, of Form I-693.

Congrats! There is a sticky here about it. Basically, you can get your vaccines transcribed by a CS on I-693 for a small fee, and not need the whole medical again, as far as I understand. Vanessa & Tony seems to be an expert on this one too; might ask her. Good luck. (Sorry, not from Canada).

It should be in that envelope you turned in at POE. The directions say to include it "IF" it wasn't done as part of the medical (because the individual consulates can decide whether to require the vaccines at that stage or not---although I don't know if there are any that don't require them). The caveat to that is sometimes they lose your entire medical. The thread that Harpa Timsah linked has advice for dealing with that if it happens to you. We had a similar issue. We did not bother going to a civil surgeon because we figured if they lost the medical, we'd get an RFE for the rest of it anyway.

6.2.2)....Will I need to take another medical exam since the medical report is only good for one year??

A...If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Regardless of if you are exempt from a medical exam, you are still required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

Some CS state you need another medical, they like to make some extra bucks. The CS I went to here in the US didn't want another medical but made me get a TB test or she wouldn't sign the 693 supplement form. Even though my medical X-ray from the CS in Canada was clear.

The whole process is just a huge money grab at every turn.

Thank you all SO MUCH :) l for the replies. So helpful and reassuring. It seems to me that the I-485 instructions indicates the worksheet from the Canada Panel Physician is fine, and the I-693 instructions also indicate that further vaccination proof is only necessary IF the DS-3025 was not done properly, or didn't make it into my packet, or my packet gets lost along the way.

I think we may just chance it, and send in the worksheet. That's a good point...if we get an RFE, that means that the rest of the medical got lost too!! Didn't think of that one.

Hello,

I just sent in my AOS package on monday. I did not get my medical transcribed, I just sent in my medical worksheet they provided me from the medical in Toronto. In the Packet I included a letter stating when I had my medical done and that I have had one done in the last year, Stated where I got it done with the address and phone numbers listed. Also, I printed out the instruction sheet from the I 485 packet regarding the medical and highlighted it..lol Just to be safe I also photocopied my reciept from dr. Seidens office as well as the chest x-ray result he gave back to me.

I guess we will see soon what the verdict will be :)

Lets hope they didn't lose our medical!!

They didn't give me my CXR report, but good idea about the receipt :)

I did not recieve a copy of the DS3025 at my medical but I did call the Dr.s office and they told me that it was included in the package that we turned in at POE. I did not get my vaccinations transcribed onto an I693 and I had no issues. As long as you have all the vaccinations that were required you should have no issues.

I considered getting mine transcribed onto the I693, but there was not a civil surgeon in my area that would do that without redoing the full medical. So I decided to save myself $300 and chance it. I am glad that I did.

I know that there are several Canadians who did not do the I-693 and had no issues.

I have a question about this. I had all my vaccinations except for the flu shot. It was not required at the time when I went for my medical (out of season). When I adjust my status, do I need to get a flu shot? Do I get a letter stating I had it done and if so, would I send that in with my worksheet from my medical?
