Confused about the documents to bring with me on the initial interview

Hello people!

I am really confused about the checklist or documents to bring with me on my interview...

Things that made me confused:

1. A completed medical examination (Form I-693) and vaccination supplement in a SEALED ENVELOPE (unless already submitted). I don't know what they meant about the Form I-693 and vaccination supplement in a SEALED ENVELOPE???? Are these the documents from St. Lukes Hospital in Manila, Philippines??? Which is the Vaccination Documentation Worksheet and a CD (I haven't watched it yet)??? When we applied for AOS on August of this year, I have already passed the Vaccination Documentation Worksheet but I don't know in the checklist if that was what they are asking for.. and what made me really confused, they are talking about a SEALED ENVELOPE...

2.All documentation estblishing your eligibility for Lawful Permanent Resident status... Can you give me an example(s)????

Hello!!! based on my understanding if u already submit that document u dont have to submit it... I will go on interview too,I just got my NOA for interview... If u have copy from the physician that u went to just bring it with u... Usually the physician gave u copy of I-693...

let's check here....did you ever have a medical exam by an authorized licensed doctor , not only vaccination documents?

If so they should have filled out this form I-693 for you and as Shescott responded given a copy to you.

Normally you should have submitted it along with your AOS package, if you didn't, there's a chance you will get a "Request for Evidence (RFE)" and/or you will just bring it along on your interview date.

As far as your question regarding SEALED goes - the form filled out by the doctor is put in an envelope which is glued closed, in most cases having additional stickers on it so it's clearly visible if you would have opened it. But you MUST NOT open it or USCIS will request a new one, plus you really should get a copy from the doctor's office for your records of it.

Hope this is understandable, if not, ask and I'll try to explain it otherways :)

edit: just re-read your post and I see that you are quoting your letter for the interview - it looks like to me that you didn't submit the form I-693 with your AOS - so you need to make an appointment as soon as possible as you will need the "results" to be handed to the officer giving you the interview. Keep in mind that they will need to do a TB skin test which requires you to go back to the office in 3 days to have the results from it. There is also a chance that they need to do a chest x-ray for that and that may take even longer......

10/17/2010 - USPS confirms delivery to USCIS

10/25/2010 - Check cashed

10/26/2010 - All applications been worked on aka touched

10/28/2010 - NOA1 Receipts for applications received

11/24/2010 - Received appointment for biometrics in the mail for 12/17 *yay*

12/17/2010 - 9.25 a.m. - 9.45 a.m. Biometrics done !!

12/23/2010 - EAD Approved and Card on its way - Email notification received *yay*

01/03/2011 - WOOOHOOO ...EAD Card in Hand!

01/04/2011 - Oh boy....Received Interview Letter today - February 3rd is the day! :)

02/03/2011 - Interview scheduled 12:45 p.m. - out of there before 1:30 p.m. - further review

Coming home, checking for touches at 5:30 p.m. - CARD PRODUCTION ORDERED !!!!

02/09/2011 - Yup another e-mail saying that my card production was ordered.

02/10/2011 - E-mail notification "Approval letter was sent today"

02/14/2011 - Welcome Letter / Approval NOA and Card (separate envelope) received!!! :D

Late November 2012 (have to look up my receipts ;) - mailed in ROC I-751

12/12/2012 - NOA 1, status extension received

12/18/2012 - Biometrics appointment letter received

01/08/2013 - Biometrics done

K-1's have their medical done in home country, the results of which are good for one year. So the OP does not need to get another test.

Since this interview is a form letter, I am going to guess that what you submitted already is fine. They send the same letter to everyone and some things don't apply to all people. I suppose there is a small chance they would give you an RFE for a vaccination supplement on I-693, but one would think if they were going to do that, then you would have gotten it by now.

Documents establishing eligibility are just your IDs. Good luck!

Thanks a lot for the response... It helps a lot... I have a CD that I got from St. Lukes hospital in Manila, Philippines...And on the CD writing, it says CHEST RADIOGRAPH... I haven't opened or watched it yet...lol Does it take too long til I get the sealed envelope??? Because I will be having my interview on January 4...

Really??? so, when is your interview... My interview will be on January 4... By the way, are you done with that form I-693???

I am still kinda confused about this... After we submitted our AOS package, we didn't get an RFE or Request for Evidence... I had my medical examination in Manila, Philippines and I thought the documents that they gave me are the documents that the USCIS needed. As what I have said from my previous message that I only got a VACCINATION DOCUMENTATION WORKSHEET and a CD which says on it, CHEST RADIOGRAPGH... and I really thought I had all the shots in Manila...

Your immunisations don't look complete to me so I would see a CS. BUT if you like you can wait until after the interview and they will RFE you there if they don't think they're complete. That way you don't spend money when it's not required..

But it says I have to bring the documents with me on my interview... I am afraid that they might cancel it for not having a complete documents... ;(
