i was born in Taiwan and got a green card and then became citizen after 4 years. how do i answer this questions?

and do i just put my us passport in the .." Have you obtained a certificate of citizenship in your name? "

Sounds like you obtained citizenship through "Naturalization". If you don't have the Naturalization Certificate, write "unknown" and then use a color copy of all pages of your US passport as evidence of US Citizenship.
