Does anyone know where I can download or get the DS-2001 form as stated in the Packet 3 from HCMC? I cannot find it. Thanks!

i have seen the form for other countries and although the content essentially is the same (it's attached to your documents and used to inform your local nvc that you're ready for an interview) the form is different per country so it's possible that so is the one for yours (vietnam). best to call your local nvc and ask them for it. also, in some nvc sites, it says it's optional to use it, so ask your nvc about this too. =)

DS-2001 is a NOTIFICATION OF APPLICANT READINESS (look at the bottom right corner of this paper, it will say DS 2001

If you go to the web site. go to immigrant visa, then go to Instruction package for K visa.

that should show you the whole package.

Thank you all for your help. I have gone to the Consulate website for the instruction package. It says they need DS-2001 and part 1 of DS-230. the DS-230 can be downloaded but there is download for the DS-2001.

Any other options or anyone know how I can get this form called DS-2001
