You really do not need anything else besides your names and made payable to The Department of Homeland Security.

You could add your address if there is room and/or put Adjustment of Status in the memo/for section if applicable.

Do I put mine (USC) or my husband's who is beneficiary.

Here are the payment instructions: http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=3850a99ba78af110VgnVCM1000004718190aRCRD&vgnextchannel=fe529c7755cb9010VgnVCM10000045f3d6a1RCRD

The cheque just needs to be written out to US Department of Homeland Security and the amount be written correctly. It doesn't matter who's name and/or address is on the cheque. If you're concerned you can write the alien number of the person applying for AOS (the USC is irrelevant) in the memo section BUT when USCIS gets the package they put a stamp on the back of the cheque that links it to the file, that's your case number.

My cheque just had "AOS" in the memo section, had my maiden name and OLD address on it. That's it. I didn't have an issue.

you put the case number or you alien registration number on the check,

No you don't. You CAN but you don't HAVE to.
