Hi,

I have been reading the packet one info on the HCMC site. Do they provide this document in the first packet? I cannot find an example of this document anywhere.

It's a single page, buried in packet 3. Basically, your fiancee just writes the case number, their name, and their address on it and sends it back with their DS-230. The DS-2001 is primarily to give notice that the beneficiary is ready to be scheduled for an interview, and an acknowledgment by the beneficiary that they know they aren't guaranteed to get a visa, and shouldn't quit their job or make travel arrangements until the visa has been issued.
