We are in the process of filling out I-693 and we noticed we do not have a DS-3025. Is it necessary? My wife did not get any vaccinations in China.

It's only required if she had her shots and wants to prove it.. If she had her shots and had a DS-3025 she might NOT need to see a CS. Obviously as your wife didn't have any she will need to go see a CS (she has no choice) and she will need to have her vaccinations checked. if she has any proof of vaccinations (like from her childhood or something) she should take them with her otherwise she will need to get all appropriate shots for her age range. Typically MMR, Varicella, and tDap... she might end up with more.

In answer to your topic question--Do all K-1 Visa's come with a DS-3025?

The DS3025 is filled out at the medical abroad and is part of the paperwork sent to the embassy and later the USCIS. The panel physicians are "supposed" to give a photocopy to the one examined before they leave the exam. Sometimes the doc may fail to give it or it gets misplaced in the move. As stated above you don't need it if there weren't any shots you want to prove with it.

The official list is here: http://www.cdc.gov/mmwr/PDF/wk/mm5901-Immunization.pdf page 2 has a table. HPV is no longer required though.

No offence to Nich-Nick but the official list (and table) is far less confusing than their post.
