What form does my sponsor fill out?

Does my sponsor fill out the I-864 (AOS) form just like me or is it an I-864A?

Unless the joint sponsor is part of your household, it's the I-864. Be sure to include evidence of their US Citizenship or permanent resident status.

Your father is living with you as the same address,so you will fill out I-864 & your dad will fill out I-864A. Here both of your income & assets will be combined.Also you need last 3 years or tax returns including W2s or IRS transcripts,copy of citizen certi,copy of passport,last 6 paystubs,job letter,bank letter(optional) and one proof which will shows that you and your dad are living at the same address.

02-28-14 -> I-130 sent to Chicago Lockbox.
03-03-14 -> USCIS received. (Priority Date)
03-09-14 -> NOA1 received.
06-04-14 -> NOA2 received. Approved.
06-18-14 -> NVC Received case.
07-03-14 -> NVC Assigned Case#(Got Case# & IIN Over the Phone).
07-22-14 -> DS-261 Completed.
07-25-14 -> AOS fee Payment.
07-29-14 -> AOS fee shows PAID.
07-30-14 -> AOS Package Sent.
07-31-14 -> AOS Package delivered.
09-15-14 -> IV fee Payment.

09-17-14 -> IV fee Shows PAID.

09-18-14 -> IV Package Sent.

See the household member test (from the I-864A instructions) below. If the father lives at a different address and did not claim the petitioner as a dependent then the father would use form I-864.

Who May Be Considered a "Household Member" for Purposes of This Form?

A relative who has the same principal residence as the sponsor and is related to the sponsor as a spouse, adult child, parent, or sibling;

A relative or other person whom the sponsor has lawfully claimed as a dependent on the sponsor's most recent Federal income tax return even if that person does not live at the same residence as the sponsor;

Sometimes, people confuses with the terms, Neal.

They will think it is my father, so he is household, and declare him as household blindly.

If his father didn't show him as dependent in the previous tax returns or his father lives at different address, I doubt his father would be under household definition.

Being 21 (or younger) doesn't exempt you from having to file tax returns. If you've worked and earned money you may have been required to file tax returns. Check at the irs.gov website if you have questions.

If you live at the same residence then your dad would complete the I-864A
