ADD FULL ADDRESSES AND LEAVE NO GAPS. LETS SAYTHAT U LIVED AT PLACE A TILL 11/30/2002 AND MOVED TO PLACE B ON 12/16/2002 BUT VISITED PLACE C FROM 11/30/2002 TO 12/15/2002. TECHNICALLY YOUR PLACE OF PERMANENT ADDRESS WAS EITHER PLACE A OR B EVEN WHEN YOU WERE VISITING PLACE C.

Your mistake was that you left gaps between dates. It is very clear from the 1st post that you have left gaps. For example :

portland Oregon usa 04-2001 05-2002

yyyyyyy yyyyy yyyy 12-1980 11-2000

Where where you in December 2000, January - March 2001? ... you see ... you there is a Gat rigth there.

Leave NO GAPS.... if you were visiting somewhere during those 4 months .. then your residence still was the previous place where you were living before.

Hope this helps
