Changing last name on my Philippine Passport

I want to get my Philippine Passport amended and change my last name on it to my married last name... so my question is, do the Philippine Passports still show your "Maiden Name" in the biographic page (the page where it shows your name, date of birth, place of birth, etc.) once you get it amended to change your maiden surname to your married surname?

I tried to google some Philippine Passport imagess to see some examples but they all just show Last, First, and Middle name.... For my middle name, will they just put my maiden name there??

For all of my immigration documents, I've already changed my surname (to my husband's) and just totally dropped my maiden name and kept my middle name. So now, I go by my (first name) (middle name) (married last name)...

Do you guys just suggest that I tell the Philippine Consulate that I DON'T want my maiden name to show... or do they automatically put your maiden name in the "middle name" part???

I believe they will do it whatever combination you want, old middle name, maiden name or both.

My wife dropped her middle name, her maiden name became her middle name & she took my last name as her last name.

Yes, it is automatic that your maiden last name will be your married middle name. And yes, you will see your last, first and middle names on your new passport. That is the DFA's rule of thumb.
