We have filled out our I-485, AP and EAD, my questions is do we need to fill out an I-693,? The medical exam was done a month ago for a K1 Visa.

Its been awhile since Ive looked at anything but from what I remember you will need just to get an appointment with a civil surgeon to get the vaccination record (the copy she got from the medical) transcribed onto a I-693....do not get fooled into thinking she needs a full medical. There may be a small fee for the transcription but much less than a full medical.

Are her shots up to date? If you know they aren't up to date or you're not sure and are not okay with a possible RFE, then she will need Part 1, Part 2 (vaccination table only) and part 5 completed by taking her DS-3025 to a CS who will tell you if and which shots she needs.

If you're sure the shots are up to date or you're not sure but you're okay with a possible RFE, then just send a copy of the DS-3025 with your AOS packet.

In the event you get an RFE, then you can see the CS then.

If you don't want to take a chance on an RFE, the VJ Guides instruct you to have a CS transcribe the vaccination records onto the appropriate part of the I-693. This is the most conservative route. Some have been approved without the 693 and some have had RFEs to have it done. If you want to guarantee no RFEs, do it. We did and no issues.
