here is my cover sheet

I am petitioning for a I-130 for my wife Marily Rago Evans

am i missing anything except for signing & dating the thing??

06/05/2010 wedding Cajidiocan, Philippines

11/17/2010 I-130 packet sent

11/23/2010 petition has been received and routed to the Vermont

Service Center for processing!!!

11/27/2010 NOA1 recieved by mail

04/23/2011 NOA2 recieved by mail....what a slow process : (

07/22/2011 AOS fee sent in also choice of address & agent form sent

08/22/2011 IV fee paid

02/28/2012 medical done ONE DAY !!!

03/14/2012 VISA APPROVED : )))

3. Form I-130: Petition for Alien Relative (my wife)

4. Copy of the full Birth certificate (front and back) for the US Citizen (myself)

5. A copy of my marriage certificate

6. a G-325A filled out by myself

7. One passport-type photo of myself attached to a sheet of paper

8. a G-325A filled out by my wife Marily Rago Evans

9. One passport-type photo of my wife

10.Evidence of a bonafide marriage which includes the following

A. 3 sworn affidavits from principal sponsers of our wedding

that took place on 06/05/2010

B. a copy of our wedding invitation

C. a wedding picture of myself & my wife on the day of out church wedding

D. I will be glad to provide any additional information including any or all of the

500 pictures that i took while visiting the Philippines for my wedding there

Our wedding sponsors included local government officials (including the mayor & his wife)

Also prominent members of the local church & community

I see that i didn't put the moneyorder on the list , could that cause a problem?

thanks my brain is starting to turn to mush with all this dam paperwork :blush:

Timeline

The cover sheet typically has a list of all items being submitted (including the check). No need for a separate table of contents.

For example see the cover letter below that my wife sent with her Citizenship application:

Dear USCIS examiner:

Enclosed please find my N-400 Application for Naturalization and supporting documents. Also, please also find a check for $ 675, covering the N-400 fee along with the biometrics fee. I am eligible for Naturalization based on being married for at least three years to my US citizen husband.

Enclosed also please find the following documents:

  • My completed form N-400 that includes two supplemental information pages.
  • Form G-1145 E-notification of Application Acceptance
  • A photocopy of both sides of my Permanent Resident Card
  • Two identical color photos.
  • A check in the amount of $675 made payable to US Department of Homeland Security.
  • A copy of the inside front cover and signature page of my husband’s current US passport.
  • My current marriage certificate.
  • Copies of two divorce decrees from my husband’s prior marriages.
  • Joint IRS tax transcripts for years 2007, 2008, and 2009.
  • Three apartment leases
  • Copies of several joint bank account statements from July 2007 to the Present.
  • A copy of my divorce decree from prior marriage.
