Child's Birthcertificate

Hello,

I have read here several times that it is a good idea to include a copy of the child of the beneficiary's birth certificate in order to prevent an RFE. However, this also leads me to a few questions

1.) Is it necessary to include the child's birth certificate in the intial petition of I-129F or no?

2.) On the birth certificate the put the mother, my fiance, under a different address, that of the father, although she did not live there. I don't want to say she lived somehwere she didn't, but don't want to coause problems on palces she lived in the past five years.

3.) Also, we sometimes refer to the baby with a nickname. Should we avoid letters with the nickname or explain this in the cover letter?

Thank you very much for you all's help, and although this is my first post, I've been reading here for a while, and the site has been a wonderful asset.
