Cover sheet for DS230 and I-864?

This a.m. I logged in and saw that our $404 has been recieved and I can now send in the DS230 and I-864. There is a "cover sheet" on the website that I printed off, does this go with both the ds230 and the i-864? We are emailing them our documents and I wanted to be sure.

Mar 24, 2011 - smile.png GC and Welcome Letter
Sept 19, 2011- Filed I-130 for my son (his step son)
June 14, 2013-APPROVED, 10 YR GC IN THE MAIL

Mar 3, 2014- N-400

May 12, 2014 Interview for Citizenship- PASSED!

June 12th, USA citizen - Oath Ceremony...all done here!!

Print out the coversheets one for AOS and the other one for the civil document(DS-230) attach each to the front of the package you will be sending.

Citizenship next step!

10-29-2010: NOA2 receive in Mail
NVC:
10-25-2010: NVC receive
10-28-2010: NVC Case # generated
10-29-2010: Update both Email I.D and IIN # receive
11-03-2010: Called NVC, still no idea about AOS BILL and DS-3032
11-08-2010: Paid AOS fee Waiting for "paid" status
11-10-2010: paid status for AOS Fee
11-11-2010: Paid IV fee waiting for "paid" status
11-15-2010: paid status for IV fee
11-17-2010: Received home appraisal
11-18-2010: Send AOS and DS-230 to NVC
01-02-2011: received interview date
