So I got my RFE in the mail today and oh boy is it soooooo confusing. I called the USCIS and they escalted my call and the guy i talked to was so helpful. This is what happend. I kept a copy of my DS 3025 vaccination record that i had done in montreal canada. Every vaccination is checked off either series completed or not age appropriate, however section 2 Results is not complete the 5 boxes that say either vaccination record complete, vacationa incomplete, individual elible for waivers etc all of these boxes are not checked off (I assume one of them has to be) So the RFE states that DS does have all your medical form in our file (yeah they didnt lose my medical) however the panel physcian that conducted your medical on june 11 2010 indicated that you did not meet all the vaccination requirments, please submit the following information.

I693 Supplement form and have it completed by a civil surgeon. They also go onto say that as of aug 1 2008 the center for disease control has new vaccine requirements for menningococal, hep a and rovisus if age appropriate if age appropriate. well on my ds 3025 clearly indicates for these vaccines not age appropriate each one of them has a check mark in the box under the age not appropriate column, plus im no doctor but i know ritovirus is for babys for diahrea, its drops you put in their ears.

The agent that i spoke to said they get a lot of calls from k1 visa applicants that receive letters saying you need to have a medical etc but yet it states if you had one within 12 months of entering the country you do not need to have a medical. I went over my DS 3025 With him and he sent a request off via email to whomever explaiing that i received this RFE but it doesnt say what vaccines are missing since my form shows no vaccines were incomplet on my ds3025 form, he gave me a confirmation # and said i should here from them within 10 busienss days regarding what they are looking for. I was concerned about not having whatever they need in by dec 9th (the deadline they gave me) and he said dont worry they wont deport you and you wont have to refile again as we have a confirmation # requesting what they were not clear on in the first place.

So the long and short of it is that I think they want me to submit my vaccine report on the I693 form. has anyone ever heard or had a request like this. I have heard and seen a lot of people that have gotten RFES for lost medical but mine who knows!!!!

Aug 23 2010 - mailed via USPS overnight courier to USCIS

Aug 24 2010 - received a USCIS at 11.40am signed for by Chyba

Sept 1 2010 - received email and text confirmation for AOS,EAD,AP

Sept 7 2010 - received hard copy of NOA1 for AOS,EAD,AP

Sept 10 2010 - received biometrics apt letter for Oct 1, attempting walkin Sept 17

Sept 17 2010 - successful walk-in biometrics apt

Sept 20 - EAD and AOS touched

Sept 27 - received email and text at 7:30pm that my AOS was transfered to CSC (yeah no interview)

Sept 29 - case touched, arrived at CSC received email and text from USCIS

Oct 4 - AOS touched

Oct 6 - AOS touched

Oct 7 - AOS touched again!!!!

Oct 14 - AOS touched again!!!

Oct 15 - AOS touched again

Oct 19 - AP approved (no email or text notification of approval)

Oct 21 - EAD approved received text and email of card production ordered.

Oct 25 - Received Hardcopy of AP in the mail

Oct 29 - Received another text that EAD card was mailed (post decision activity)

Nov 1 - Received EAD card in the mail

Nov 9 - RFE for vaccination record transcribed over to I693 form

Nov 13 - Found Dr. who would complete just the vaccination part of the form yeaaaah!!!

Nov 17 - USCIS received my RFE response papers in California

Dec 3 - Received email and text confirmation that my case was updated from RFE to RFE review response

Dec 9 - Green Card approved, never received any email or text and my USCIS case still shows in RFE Response review

Dec 14 - Received my approval notice and welcome to America letter advising it was approved Dec 9th

Dec 17 - Green card received, finally done with USCIS for 2 years

I cannot comment on what to do as i am a cr-1 and I just poed a few weeks ago but I know that on my copy of the vaccination record ,the toronto medical doctor left all those boxes at the bottom unchecked as well. I know this because I checked the sheet before going to my interview and I thought oh ####### do not tell me that the doctor was suppose to check them off. I am sorry you are in this position but maybe it is something else?

Yes a lot of people have gotten RFE's for not getting the I693 Supplement form transcribed and have it completed by a civil surgeon. You are supposed to do it whether you are a K1 visa or not. Its hit or miss....sometimes they let it go (they did in my case and many others) but some send out an RFE for it. I would start calling civil surgeons in your area and get some prices for having it transcribed. You do NOT need a medical yet some of them don't know that and will try to make you do it. Also do a search on the boards....many people have posted different civil surgeons in different areas of the country that do it for a reasonable cost. The faster you just get it done and send it in the faster they will resume processing on your case. I've read some of the civil surgeons will do it for as low as 10-25 dollars.

I think your RFE is because the panel physician did not properly complete the DS-3025 by checking a box in Results. It's required.

You can read the technical instructions for completing the form starting on page 8 (p. 12 of the pdf) http://www.cdc.gov/immigrantrefugeehealth/pdf/2009-ti-vaccination.pdf

On page 9, it tells how they are to complete the Results section. "After reviewing entries in "Part 1. Immunization Record" for all the vaccines, the appropriate box under "Part 2. Results" must be checked."

To correct that now in the US, you will have to get a civil surgeon to copy your immunizations to an I-693. It would be easier than trying to convince the doctor in Montreal he's not following the instructions and you want it corrected.

I think your RFE is because the panel physician did not properly complete the DS-3025 by checking a box in Results. It's required.

Well I talked to my doctor that did my medical in montreal and they said that my vaccines were all up todate, due to my age the only ones i need are MMR, Tetnus and VH and i had them, but the issue is that he forgot to check off the box that my vaccines were complete. He apologized and said he would create a new DS 3025 form for me and call the Consulate in Montreal to see what he has to do. He called me back within half hour and said that Mtl Consulate said that I Should have a civil surgeon complete the I693 form because they can not speak for the California Service Center. Well i called a civil surgeon that has an office a block from my work and they said they would transcribe my vaccine record over for $30 and have itready for me tomorrow to send off to the UCSIS California Center. I hope that this is it for RFES for me its annoying. I assume that they do check your case and tell you in one RFE what you are missing not ask me for my medical, then check another form and say oh that is missing etc, please tell me they are some what efficient!!!

You are thoroughly confused.

The RFE you received clearly states they HAVE your medical but that your DS-3025 indicates your vaccinations are not up to date. Therefore you see a CS, get a vaccination transcription and done. USCIS hasn't said anything confusing in the RFE that you transposed.. although you should use paragraphs when posting because your posts are very confusing.

It appears you've sorted the vaccination issue but still seem to think USCIS was confused or the RFE was confusing.. it wasn't and isn't according to what you posted.
