Your wife will get Visa without any hassel, she will be able to travel and also get a Green Card with no trouble.

But the name on Green Card and Social will be per Passport.

In USA, you have freedom to keep any middle name and last name regardless of family, even you can opt to not to keep middle name.

Problems you will face if you choose to change your wife's name (I mean last name) after 5 years when she becomes US citizen and apply for Indian OCI Visa. Indian consulate will ask you to prove that the person shown in Indian Passport and New American Passport is the same.

My friend has the same issue, and he has to file affidavite and post name change ad. in local indian news paper (standard name change process in India) because of exactly same issue.

FYI, now Indian consulate in USA and Regional Passport Offices in India do have a specific form and procedure to change woman name after marriage,, Also you can change name on Social record, I have no idea for Green Card name change.
