Medical Examination - When?

Timeline

So I'm unsure if anyone has started something like this. But I haven't been able to find a straight forward answer.

I have a question in regards to WHEN exactly you schedule your Medical Examination. I am a bit confused. For reference we have submitted my IV package already.

Now I'm wondering when I get the medical exam done. Do I wait till I hear from them when we get our interview information? Or can I plan ahead and get the medical examination done now. I'd be getting it done in Ontario (Toronto to be exact = As that's the only certified doctor on the consulates PDF list)

Thank you in advance and I'm terribly sorry if this is always asked.

03/04/2010: Submitted I-130

06/29/2010: NOA2

NVC

09/02/2010: 1-864 & IV Paid

10/16/2010: 1-864 delivered to NVC

10/27/2010: IV Package delivered to NVC

11/18/2010: AVR "Case Complete" at the NVC

02/14/2011: Medical

02/23/2011: Interview APPROVED!!!!

03/04/2011: Visa Recieved

03/04/2011: POE: Lewiston Bridge, Buffalo, NY

ROC

01/12/2013: I-751 Packet Submitted

01/14/2013: I-751 Packet Received (via certified mail)

01/18/2013: NOA1 Letter Received

Also I'm in confusion about the photos. On the Medical Examination PDF from the Consulate General it says 3 photos and on LingChe's Visa guide it says 2 passport photos.
