Hello all,

I have an appointment today with my vaccine transcription at 12. I just want to ask if the CS already know what to give regarding the sealed envelope or do i have to ask him to do that and show that statement from the I-693 form? I made one copy of I-693 form , do i have to make another one?

I need some ideas and help, thanks and God Bless!!!

they know how to prepare the package for submission.... they should have the forms as well

Take the 693 with you filled in with your info on part 1,. Take 2 and 5 and also your vaccination supplement from your medical. They will do the rest and give back to you in envelope. They will also give you a copy to keep for your records as well and if they don't ask for one.
