Question #11 "Have you ever applied for work authorization from the USCIS before?"

I previously held TN Status from 2006 - 2009. Two were POE issued and one was applied via Vermont Service Center.

Therefore, given the above, do I reply YES and explain myself for all dates or are the ones I received at my POE's not counted for in this case? It further asks for documentation, but I have none! My documents were misplaced when my husband moved us into our current home. I have my old passport with my last TN issue on it that I can copy but that is all. So, will that be sufficient or is there a way to gather the previously issued information from the USCIS?
