Lots of questions on if Medical Info is needed. We did not have DS 3025 (go figure!), so I went ahead and did Full Medical Exam for $200 in Rowland Heights, CA.

Regardless you will need to send in I-693 and only Civil Surgeon on the "list" can fill them out - this was "read" on the I-485 instructions.

Where to send:- If using plain old snail mail use the PO BOX, else for US Express Mail service and other carriers use street address.

How long to wait? We waited 1 week before receiving electronic acknowledgement from USCIS that they receive our app and another week to get NOAs/797's.

You will have two NOAs - one for the I-485(AOS) and the other for I-765(EAD) and hence 2 case numbers as well.
