Toronto Medical Vaccination Record for AOS

Alrighty, so my husband and I were finally married on October 10th, and we're getting things together for AOS. I was looking through all of our paperwork, and I'm curious about the "Vaccination Documentation Medical Worksheet Two" (title of the form) that he received from Dr. Seidan's. Nowhere on it states that it is the DS-3025 that we're supposed to include, so I'm just wondering if this is it, or if we need to take this somewhere else and have it transcribed. He's 100% current on any and all shots he needs for immigration, and this form appears to be filled out with dates and/or "not age appropriate", but I definitely do NOT want a RFE. We also have his shot record booklet thing that's been filled out completely, too. I'm hoping that those who have gone through Dr. Seidan's will know which form I'm talking about and be able to answer...

Some people have gotten away with just sending in the worksheet. We only sent the worksheet in, but ended up taking the worksheet to a civil surgeon before the interview and having it transcribed just in case. Our interview officer told us that it was new policy that they were required to ask for it at the interview if it wasn't included in the submission. I have however heard otherwise from others on here who subsequently went through their AoS interview and were not required to have the I-693 transcription.

There is no "set rate" for the service and some of them charge WAAAY more than others. I got mine done for $30, but other places told me it would cost $140.

The "Vaccination Documentation Medical Worksheet Two" is what you're supposed to bring to a civil surgeon to have transcribed. I would suggest calling around to a few different Civil Surgeons. There is no "set rate" for the service and some of them charge WAAAY more than others. I got mine done for $30, but other places told me it would cost $140.

I just got an email that I'll be getting an RFE for my application. I'm betting it's for the I-693, because all I sent in was the "Vaccination Documentation Medical Worksheet Two" (which I thought was the same thing as the DS-3025) along with a photocopy of the requirements for K1 Visa holders, which states that they don't need a new medical as long as they have a current medical on file.

We also went to Dr. Seidon in Toronto.

I'll let you know what the RFE says, if it is related to that. I assume the RFE will come in the mail next week.

I used Dr. Seiden in Toronto and just sent the worksheet in with my AOS. I was approved at interview without needing the I-693.

With that said, if you "definitely do NOT want an RFE" I would have the worksheet transcribed onto an I-693 by a US civil surgeon. If you do not, you will risk getting an RFE. Some do, some don't.

Actually, that was supposed to be Dr. Arnold in Austin, TX - not Dr. Austin. However, same difference -he/she (no one was really sure of the good doctor's gender) is no longer doing immigration medicals.

Here is the USCIS link to their 'Find a Civil Surgeon" locator. You just type in your zipcode and it will print out the list of doctors in your area.

That's exactly what I did. I called 12 doctors in the Denver metro and none of them would do it. It was ridiculously frustrating! Nobody knew what I was talking about, and nobody wanted to listed. I got comments ranging from "a foreign medical is not valid for AOS" to "There is no such thing as a K-1 visa" I even got a quote from one doctor who wanted to redo my medical and charge $850.00.

Update from us: We decided not to risk it, so we started calling around to the CS in our area. Most places wanted $200 or more, because they "don't do just the transcribing" and he "had to have another medical". RIDICULOUS! Luckily, one of the places he called will do it for $10, and his appointment is in 2 weeks. PHEW.

We took the chance and didn't do the transcribing. So far no rfe.

I reviewed my entire AOS packet this weekend- couldn't find one mistake. So now I am waiting for the RFE, and thinking it's got to be the medical because what else could it be? I am expecting it in the mail any day now . . .
