Before calling some CS in my area, I did a lot of reading regarding the I-693. Looked through the list of CS in the USCIS website, there are only 2 places in downtown Toledo. The rest are like 30 min. drive from my house.

Had an appointment in St. Vincent the other day. Went to the Occupational Health Dept. The receptionist asked me to fill out some forms and papers, one of them is letting them perform a Physical Examination, but I told her I won't be signing it cuz I don't need it. Waited for a little bit then heard my name called by a nurse. Once inside, she told me that a CS won't sign my I-693 if I won't have a physical exam test and TB skin test. I showed her the page in the I-693 instructions about not having to have another physical exam anymore cuz I just had my medical last January. Still, she told me that they should need a proof that I had my medical in my country. My husband asked how much it would cost us if I would have another exam, and she said that without the DS 3025, it would be $300, however, I already have my shots, so it will only cost us $150. My husband then told her that we will just have to go back cuz I will be making some calls cuz of the conflicting information that we get. She then said that if we really want to only have the transcription, she can do it right away, but I said no. Before we left, she gave us a piece of paper and told us to call and asked them if I really need to have a physical exam or not.

That afternoon, I called the International Institute in Toledo and asked if I need to have a physical exam, and the lady in-charge said yes. I explained to her about the instructions, and she said it doesn't matter. I was getting confused and thought of calling the USCIS just to make sure. Was able to talk to a customer service rep. after waiting in line for almost half hour, but like what I already know, it's only the transcription of my vaccination records, and if I need to have another shot, so be it, but no physical exam anymore.

Yesterday, I called the hospital to set another appointment and told them that I talked to a person from the USCIS. Today, we went there and the receptionist (a different one) told me that I should fill out forms again using my maiden name because the name in my passport is still my maiden name. I explained to her the situation but she didn't listen, she still wanted me to use my maiden name and had me fill out another I-693 (I had mine filled out and printed at home), so I had to do what she said. I was thinking that we would go to the same nurse in my appointment the other day, but then the nurse today was a lot more knowledgeable about the process. She had me sign the I-693 that I brought and praised me for being really prepared (she said a lot of applicants would go there empty handed and didn't know what they would do). She said that I won't be needing another physical exam and tb skin test. She was really good and explained everything to us. Finally, she said that everything was fine and no need for me to have another shot because the CS told her that the MMR can be one or 2 shots (I had one back in Manila). We only had to pay $29 for the transcription.
