So hoping someone can clear this up for me.

I've got the fiancee visa (yay) I fly out in november, but with all the changes to rules in regards to flying to the USA are there any forms I have to fill out before they let me board the plan.

I know if you fly on the visa waiver scheme you have to apply for a ESTA, but what if you have a visa???

I will flying out from London.

You just need the envelope the Consulate gave you for the Immigration at the POE. And that's it... Book your flight and you're good to go.

Maybe someone from UK could shed a better light than me on this one. :P But I think you don't need to fill in any forms.

NO ESTA required...

Fill out the I-94 (white form) and customs declaration on the plane

From the UK and agree. I will add, a guy checking the line at Heathrow seems to ask everybody, "do you have your ESTA?" Just say, "No, I'm entering on a visa" and he will probably look at it. The Continental counter person actually asked if he had his brown envelope with him for carry-on. She said she was just checking so he wouldn't have problems on arrival.

Thanks, but the last time I flew (August 2010) no one was giving out the white form-just the custom form. Is that the same thing?

Here's the immigration forms, called Arrival/Departure Form. I-94W is green and I-94 is white. See examples here.

They don't give out the I-94W anymore since ESTA is finally paperless since around August, but I don't know what they do about the I-94 on the plane. You will need the I-94. When we flew from Heathrow in June, there were piles of both forms on the airline counter, so you might ask when you check in. And you can ask on the plane for one. And when you arrive, the immigration people will have one if you didn't get it earlier.

The Customs Declaration form you will also need has a blue border. See sample on this page.
