Incorrect Last Name in USCIS Database

So my wife and I successfully adjusted her status from a K-1 visa to a permanent resident. We received her greencard on 9/25/2010. Everything is correct on the greencard, however, when we went to the Social Security office yesterday to get her a new card with her new last name (my last name), we were informed that her last name in the USCIS database is still her maiden name. Fortunately, the lady that helped us was able to process the new social security card but she warned us that this may cause some issue in the future especially when we go out of the country.

Has this ever happened to anyone? How do we go about fixing this problem?

I had an issue almost like this...they had my First and Middle name both together as my first name...was funny but this is what they had it as and it was a matter of contacting USCIS and telling them to correct it. I believe you may have to submit the Marriage Cert as proof.
