My fiance seems to have misplaced the immunization form the doctor filled out at his medical last month. He made a couple photocopies, but now he can't find the original.

When he applies for AOS will we need to present the original or will a copy be OK?

You need to submit the I-693A Vaccination Supplement with the AOS application. When my husband got his, he had his vaccination record from his doctor (his mom got it and mailed it to him) and this is what we took with us to the CS. They put it in a sealed envelope and we submitted it with our AOS application. If you don't have your SO's Imm. Record, you'll have to get one to have this form filled out by a civil surgeon. Maybe someone else can add to this, or has better advice. Best of luck :)

If he has a photocopy of the vaccination form (DS-3025) from his K1 medical exam, that is good enough. I think the original goes in the mysterious brown envelope that is turned over at the POE. You can take the photocopy of DS-3025 to a civil surgeon, and they will use it to fill out I-693A; that's what my husband and I did.

Also -- full disclosure -- some folks have been successful in filing for AOS with a copy of DS-3025 instead of going to a civil surgeon to get I-693A filled out, but only if all vaccinations were complete at the time of the K1 medical examination. But there is also a risk that you will get an RFE for failing to file I-693A, since USCIS guidelines clearly state it is required. It all depends on who is reviewing your AOS package. One poster here got an RFE at her AOS interview for not submitting I-693A; she had submitted DS-3025, which showed that all her vaccinations were complete. So there is some risk involved, and I would recommend getting I-693A filled out by a civil surgeon even if your vaccinations are complete on DS-3025, but there are plenty of others here who advocate just using DS-3025 (if it shows all vaccinations completed).

KarenCee's post just reminded me of an issue with our I-693A. We got it done at the civil surgeon, they had no idea what was going on, and kept writing down vaccinations that i didn't need, not listening to what i really did need, etc etc. :bonk: The civil surgeon did not give us our I-693A in a sealed envelope. Is this bad?? Should we be cheeky and simply put it in a white envelope ourselves and seal it up?? or leave it as is, and it's no big deal not to have it sealed??

The I-693A must be sealed in an envelope and the civil surgeons office must stamp along the seal so after going to the effort of getting the form filled in and no doubt paying the money for them to do it I would take it back to them and get them to seal it according to the correct procedures. Or you could end up like me with an RFE at my interview.

Jum's documents in that magical brown "Do not open" envelope were taken by immigration at her point of entry. Therefore the orginal immunization record was in there. I contacted the original hospital where she had her immunizations, they sent her a scanned copy via email. Hopefully that will work for the AOS package. We'll see.

My original immunisation record was also in the magical do not open envelope given to immigration at my atlanta point of entry and I just had a copy of it to take along to my interview. Since all my immunisations were completed I thought that everything would be fine what a mistake. After our RFE at our interview we went straight to a civil surgeon and had the form transferred onto a I-693A then sent it off with the envelope we were handed at our interview. So now we are in LIMBO not knowing when I might get a green card or a welcome to america letter.

Just to let you know, I'm in pretty much the exact same situation myself now (trying to get hold of a copy of the immunization record from the medical practice). What I'm going to do is submit a copy of the record with my AOS application, and keep a copy for myself. I'll then get a civil surgeon to fill in the I-693A and take this to my AOS interview, just in case. :-)
