RFE looking for I-693, but I sent the DS3025

Hi - the exact same thing happened to me. I made an infopass appointment, showed them photocopies of my completed DS3025 and they corrected the mistake. No cost involved.

Oh, I need to remember that as one of the options I listed earlier. I always forget the INFOpass appointments. :(

June 4, 2009: Interview - APPROVED!!!

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

we still had to fill out the form (part of it) and have a civil surgeon transfer information from vaccination record to a form required by the uscis......they accepted it all.

10/04/2010 recieved RFE, forgot to sign a document. Signed and returned.

Here may by why some get an RFE and others don't. The rules states, "Only if your DS-3025 is marked "incomplete" is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US."

Food for thought.

We are applying for our AOS in a few weeks and will be checking our form before submitting.

Can you help please?

I have applied for a I-485 after obtaining a K1 successfully, as I had completed a medical (March 2010 in Ireland) before moving I supplied just a copy of the DS3025 with my submission, no I-693. I also handed the originals in at the POE (inc. x-ray).

My understanding is as follows..

If the beneficiary's vaccinations were COMPLETED as part of the visa process, IT IS NOT NECESSARY TO INCLUDE FORM I-693 IN THE AOS SUBMISSION. All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received. If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693.

I received an RFE saying they don't have any medical records on me (including a DS3025) and want me to submit an I-693.

Is this just a case of a lost document and I send them another copy of the DS3025 or am I missing something?

And.....this is why the VJ Guides state that even if you have the 3025 to ALSO have a CS transcribe the vac. records onto that portion of the I-693. We did that and no problems. I always try to stack the deck in my favor.

Some people are missing the reason for the original poster's RFE. It wasn't about immunizations and the DS-3025. It was because USCIS said they had no record at all of the medical results from overseas. The files were not matched up with the AOS file. With NO medical records they can't approve. So he can suck it up and take the whole medical exam again or he can try to get them to find the brown envelope he turned in at POE. Some times a congressman can wake them up to the fact that there are indeed medical records. Maybe an infopss can wake them up. Maybe the person opening the packet was a new rookie contractor and didn't know to order the A-file to get a K1s medical report. Most AOS applications arrive with a new whole medical, so rookie contractor said "no medical--must send RFE. K1s require that extra step to get their medical into the AOS file.

And like h1a1 above, our DS3025 was marked "incomplete, may be eligible for blanket waiver" and the GC was issued with no I-693 or RFE. But that's not the real topic of the RFE experience that started this thread. Our medical files were not "lost."
