For my benificiary it states her name, her birthdate, but under Country of Birth it states UNKNOWN. Is this common? I mean other than the G-325A, where it clearly states her birthplace and as well as her BIRTH CERTIFICATE that was provided, shouldnt it state on the NOA1? Please tell me this is a common thing.

I just checked and on my NOA1 it didn't even have a slot for "country of birth". ?

The NOA1 is just a receipt. Never to used ever in the future process, ignore it.

There were a few people including myself that had the same mistake on there NOA1 form. Some called to have it fixed, some just left it. Personally I think making sure her name and birth date are correct is more important that the country of birth. But if you want total piece of mind then feel free to call.
