I-693 CS Vac Record (blank boxes on form) ?

I have my I-693 supplment from the Civil Surgeon. All the medical vaccination record is there as it should be.

I have a question about all the blank boxes left on the form. That is stuff dealing with what was done during the medical.

I saw an example form where someone had written in N/A in all the blank boxes and also "N/A - K1 applicant completed Consular Medical in the last 12 months".

I don't see an example here in the guides. I'm assuming it's ok to do that to not leave any pages with blank boxes ?

K1 timeline :

1/11/10 - I-129F sent to California Service Center

1/19/10 - NOA1

2/18/10 - moved and changed address on USCIS site

2/19/10 - touched

3/10/10 - touched

3/10/10 - NOA2, hardcopy recieved 3/12

3/16/10 - Left NVC, recieved MNL case #

3/22/10 - USEM recieved

4/19/10 - Passed Medical

4/28/10 - Interview - Approved

4/30/10 - Picked up Visa and completed CFO

5/5/10 - POE - Honolulu, Hawaii

5/6/10 - married in Hawaii

AOS timeline:

7/8/10 - received SSN Card

7/16/10 - reported US marriage for RP records

10/18/10 - AOS delivered in Chicago

10/25/10 - NOA1

11/04/10 - Biometrics Letter Received

11/23/10 - Biometrics Completed

11/24/10 - touched

11/26/10 - touched

02/14/11 - AOS approved at Interview, GC ordered

02/22/11 - GC arrives in mail

ROC Timeline:

12/17/12 - I-751 sent to California Service Center

12/20/12 - NOA1 arrives in mail

1/14/13 - Biometrics appointment

4/11/13 - RC Approved at Interview

4/22/13 - 10 Year Green Card arrived in mail

Timeline

First. You only need to include Part 1, Part 2 (vaccinations only) and Part 5. Your medical should be in a sealed envelope but you can ask the CS for a copy.

I was approved with no RFE's and no interview. Hope that helps.

Did you mean the vaccination record is in a sealed envelope ? They gave me a sealed envelope and a copy. Only USCIS has the medical record sent from St. Lukes right ?

You didn't include the sealed envelope when you submitted did you ?

I mean your vaccination transcription which is medical form I-693 (sorry should have written that)... the impt part being at least 3 pages should be in the envelope, not JUST the vaccination page. I have a copy of EXACTLY what's in the envelope (except my copy isn't signed so i can't copy his signature)

I thought I would hold on to the sealed envelope in case it was asked for in the future like at an AOS interview if there was one.

Either you or I are confused here so I'll explain further.

When you went to the CS s/he filled in the I-693. It would only have been 3 pages (part 1, part 2 (vaccinations only) and part 5). S/he then would have sealed a signed copy in the envelope and given you a copy for your records (my copy for my records was unsigned so that I didn't give it to someone else or something).

When you send your AOS package you send the sealed envelope the CS gave you, and hold onto the copies for your records (in case there's an interview, or they lose them etc).

from your reply you said that you needed the 3 parts from the I-693 form that was just filled out by the CS. The other pages with blank boxes weren't needed. That was my first question.

Then you said you sent in the sealed envelope.

So right now I'm sending copies of the form that I have with the filled out portion, leaving the blank pages out and just attaching the sealed envelope.

I'm assuming that should be fine.
