transcription of vaccination record

hi everyone ,on our way to put things together for aos...i am fromt he philippines and had my vaccination record from st.luke's my question is do i need to have my vaccination record transcribe before submitting it to the uscis???anyone can shed a light on these.just confused cos all i know is you just only need to have the record transcribe if its not in english.any inputs will be appreciated

Yes, you must go to a USCIS doctor who will look at your vacc. record and will give you any shots you may need. After that, the doctor will sign the I-693. You will send this document (with the other docs too) to the USCIC for your AOS. If you do not get the I-693 signed, you risk an RFE and a lot of wasted time.

Well you MAY HAVE gotten all the shots in the Phils. That is why you need to have the approved doctor review your records and then sign the form. The office workers at the USCIS have no idea what shots are required so they require a doctor to look and see if you need any. Some shots take 2 or 3 boosters for the full effect. My wife needed 3 shots to finish her medical immunizations.

Make sure you pay special attention to the dates you had the required shots - tetanus with 5 years and MMR within 10 years. If that is not the case you will have to get those shots before going to the civil surgeon (or get them done at the civil surgeon) you will receive a new sealed envelope that will go with you AoS package :) along with a vaccination copy for your records.

Make sure you pay special attention to the dates you had the required shots - tetanus with 5 years and MMR within 10 years. Unfortunately this isn't the case. USCIS aren't trained to know the intervals for shots so that's why a CS needs to sign off on it IF your DS-3025 says "history incomplete". If you DS-3025 says "complete" you are fine to send as is. Otherwise see a CS for peace of mind because you'll most likely get an RFE unless you're lucky enough to get someone who DOES know the intervals.

I do believe we are saying the same thing :) in order to get it transcribed you must see a civil surgeon :) they will let you know but you can also be prepared. Complete (as mine was marked in Canada) with a MMR shot in 1983 was not sufficient for the I-693 you have to have one more recently. If you are unsure whether or not you will need another vaccination, you can be certain the the civil surgeon will let you know :)

The best way for you to find out is to call the USCIS. In my case since my medical is more than a year I called the USCIS and verify if I need to get another medical exam and they said I need to get a new one. They will help you find a dr. that is USCIS certified to perform the exam. I contacted the dr. and set up an appointment (he will tell you what to bring). I was expecting to spend a lot of money for it but I was surprise when I showed my vaccination record and he said I dont need any vaccination because mine is complete. So he did physical exam, he sent me to do a blood work for HIV etc, x-ray. I pent $320 in total.

Then got my result after 1 week. You should bring your vaccination record from St. Lukes, passport, and I-693 form. And oh this will be helpful;

I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. Only if your DS-3025 is marked "incomplete" is a I-693 required to be completed. If it is marked as such (incomplete) you are required to complete certain portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically, you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations that were completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on re-administering all the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, they will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you

That's the theory. I say theory because USCIS tends to stuff things up. To be safe I would see a CS. I paid $50 for mine and I had peace of mind when filing.

I just got off the phone a few minutes ago with USCIS (800-375-5283) about this very question.

My wife received her medical exam four months ago. When she received her DS-3025 form, it was marked "Vaccine history complete for each vaccine, all requirements met" in the lower left corner.

I was told that the I-693 did not needed to be included in our AOS package, but we would need to provide a copy of the DS-3025 form.

I got my vaccination transcribed by USCIS Surgeon within our area and they really make our pocket empty..hehe.We paid 90USD to transcribed and got the sealed Envelop. In my DS-3025 it was vividly stated that i need to do shot for TD/TDAP as this vaccines is not routinely available in KL and the doctor explained to me that I need to do shot in the US for this vaccine..Thanks to Goodness!! and so lucky that my husband Medical insurance included me right after the marriage..so timely I can used the Medical Insurance so its free to have the shot and I also make sure the chicken pox shot also included ( its free) eventhough it was stated in my DS-3025 as VH ( means I had my chicken pox) just to avoid any problem later. I took the list of shots from the clinic to the USCIS- doctors to do the transcribed and I waited less than an hour and I got the sealed Envelop = 90USD:-))))...

Oh yeah,Indeed, I am luckier then;-), I thought they charged me too much as some Vj info they only paid 20-50USD for transcribed.
