I am the petitioner going to fill out DS-156 for my fiance, should I include my name on questions 39 and 40 "Application Prepared By" and "Was this Application Prepared by Another Person on Your Behalf"? I was reading mix responses from the forums and some answers were not clear at all. So I do need your correct answer, please.

I have received the instruction from embassy stating following regarding this document:

One non-immigrant fiancée form DS -156 K: This form must be completed by K1 applicants only. NOTE: Please do not sign this form.

if you fill out the DS-156 then you can help them prepare it and then fill out that part however the DS-156K must be prepared by the beneficiary only

good luck

What Tyeger said.

I explained it in your other thread a little more as I am in a similar situation.

Fiancees printer doesnt work, so I gave her the PDFs to look at, and she told me what to put into each one (in English since she speaks it very well) and I would print it. (The PDFs are read only -_- so she couldn't type in the info and send it to me to print). So in my instance I wouldn't sign that area as all I did was print them, I didn't help her fill it in.

But for you, you would need to because the counsulate could question how she filled it in, since as you said, her English isn't very good. It doesn't slow the process down at all.
