DS-3025 Vaccination form Incomplete

Dear all,

I have a question concerning my DS-3025 vaccination form the panel physician filled before my appointment to have the K-1 visa.

I received my K-1 visa.

But I had a look on the DS-3025. it is incomplete because I had only one vaccination for the MMR in my life (I made it just before my medical appointment).

It is required to have this vaccination twice in your life. My doctor wrote in my vaccination record that I have to make the other one in 2020. The panel physician could not my the second one because you need several months between the both vaccination.

1/ My question is, when I will be filing for my adjustment of status (i-485), will I have file a I-693 ?( so to go to see civil surgeon)

2/ Will they tell me at the border that I need to do it ?

The people at the border are highly unlikely to know anything about anything.

If you are required to have another MMR vaccine, you will only have to have the civil surgeon give you that one vaccine and fill out parts 1, 2, and 5 of I-693.

You may not need it though, as it might be considered complete as it says to have the next one in 2020. As it is possible for K1 visa applicants to not do the DS-3025 until they after they immigrate, it might be that you only need to submit it if you didn't initially. On the one hand, medically you could have the second MMR by the time you file your I-485; on the other hand the doctor wrote 2020 so you might be fine.

Out of interest (and without knowing your medical contraindications) an MMR vaccine can be given again after the first dose as early as just one month.. so I'm not sure why your doc wrote 2020...

To avoid an RFE you would be wise to see a CS to fill out Part 1, Part 2 (vaccination table ONLY), and Part 5. Usually this will cost about $50 (hopefully less, definitely shouldn't be more but some docs rip you off). Even though I KNEW my shots were 100% complete, as my form was marked incomplete, I went and saw a CS and had my vaccinations transcribed just to be safe...

And no, they won't ask you at the border about it.

1. Yes. They don't complete any other part of Page 2, only the vaccination table. It's a process called "vaccination transcription" and that's what you should tell them when you call. If they tell you you need a TB skin test you don't need that either. It's part of the medical and you aren't having a medical.

2. No. You said your panel physician wrote 2020 so you could hope that this means you might not need it. I wouldn't want to get a shot you don't need if you don't have to. Hopefully save some money.

3. there is no prescription for the shot. You would go, give them your info to complete part 1 of the form, they would read your vaccinations and decide what you need and either give you the shot there and complete your paperwork OR ask you to come back while they complete the paperwork and give you the shot then... my CS did the second option, made me come back to collect the completed paperwork (I didn't need any shots). The cost should just be standard transcription fee (that's to complete the paperwork) and then the cost for however many shots you need.

According to the panel doctor who performed my K1 checkup and a nurse at my CS' office, doctors usually give a 10 year period between MMR vaccines. It can be given sooner, of course, but the previous dose is still effective and therefore a second shot isn't needed till much later. That's what I got from their explanation anyways.

You didn't mention Tdap or Varicella. Those are also needed. If you had chickenpox, the panel physician may have written VH (varicella history) on the line by Varicella. If you have the VH written, then you don't need that shot.

Hi,

Wondered if somebody might be able to clear up something related to the above thread. I am currently adjusting status but when I had my medical examination in London I opted to have a required MMR vaccination done in the US. Am I correct in thinking I must complete the I-693 now and book an appointment with a civil surgeon to complete this and have the vaccination before I can submit my adjustment of status documents, or can this be done at another point. Sorry if I come across naive as there is plenty on the forum about this but just a little confused/anxious to get everything sent off without an RFE.

Yes, you are correct. You must get the missing shot and then get your vaccinations transcribed to I-693. You must submit it with your AOS packet. If you don't, you will get RFE which will cause a delay so it's better to submit everything together.

We went through the same. At Poe, the officer took my wife's

Sealed medical envelop along with other papers provided by the embassy. Poe is going to mail it to USCIS. If you file within one year of your medical exam, you don't need to see a civil surgeon. My wife didn't. Her aos just got approved.

Forgot to mention that my wife's also showed incomplete because of future shots that couldnot be given at the time. If your are do for your shot between your arrival to the us and the time you file, then you should see a CS. If not (which we didn't) you do have to. USCIS should have all of your K1 med papers from the POE.
