My k1 visa interview took place in Islamabad U.S embassy on Oct 05, 2009 its almost 12 months now, recently my fiancee made a phone call to department of states and asked why case is taking so much time, lady told that our case is in admin processing b/c one document is missing which is "I-134 Affidavit of support".

I was not sure about this document when i sent my documents to U.S embassy in Islamabad, further she told that we should have given you visa last year, if you provided this document at the time of interview.

This statement was shocking for me.

I am wondering i have been make call around 1000 times to U.S embassy Islamabad and they never told our case is in admin process due to this document.

Any one from Pakistan submitted their k1 case without "I-134 Affidavit of support". and they got visa ?

There may be some confusion here, bear with me as I try to illuminate the possibilities of what may have happened. You state you are not sure if the affidavit of support was submitted to the embassy. IF this is really the only thing holding up your visa it is a very big mistake on the part of the embassy as they should have told you at the time of the interview that you needed to provide this document. BUT here is what I think really happened.

You interviewed and submitted all the documents. Being as you are a male from a T country you have administrative processing, no way around it. Because your case has been almost a year now and probably more than a year since the documents were prepared they are NOW out of date. So although the affidavit of support may not have been the reason for the AP because it is more than one year old it is now considered not valid and a new one needs to be submitted.

What you need to do is contact DOS and find out if the AP is completed. I would really at this point have a congressman involved. Because your case is about one year old all of your documents are now out-of-date such as the medical exam, the application for the visa, the affidavit of support, etc and all new ones will have to be submitted BUT I would not worry about doing this until you are sure the AP is complete.

So have the US citizen call DOS and you call the embassy. Try to get in touch with someone with some knowledge and not just the person who answers the phone. Specifically ask if your case is out of administrative processing and exactly what you need to submit/do at this point.
