The newer form does not require a notary. Be sure to get the latest version from the USCIS website. There are a lot of old forms floating around various websites and it is easy to forget and download and send in an antiquated form. The older versions of the I-134 form DO REQUIRE a notary.

I got lucky (maybe blessed?) when at the very last review I did before sending in my 129F package last year. I discovered a form that was 5 years out of date! Saved me a RFE.

u dont need to just use the downloadable forms here at vj and u will notice at the end of the form there was an oath or affirmation of the petitioner secure a signature from ur petioner and the date and u will be fine Godbless:D
