Hey all,

Almost done putting my application together and I have a few questions that I am not 100% confident with my answers.

Can you please advise?

1) Part 1 - B "Name exactly as it appears on the card"

So on the card it shows my name under the name section as FIRST NAME/LAST NAME/MIDDLE INITIAL and at the very bottom it lists my full name, which is too long and it doesn't even fit the card. Are they asking for what it shows next to the "Name" section of the card?

2) Part 4 - C "Telephone and Email"

Do I need to give USCIS email and phone number? Do you all do that?

3) Part 5 - D and E

I was born in Brazil and this question always makes me confused. Should I check YES or NO for the "Are you Hispanic or Latino" question?

Also, by marking YES, do I still need to answer the question below ("Race")? I am assuming I should say either yes or no for the Hispanic question and then mark "white" for "E". WTYT?

4) Part 10 - B "Affiliations - Have you ever been a member of...."?

So how ####### are they with is? I haven't been a member of any organization, association, society, etc... but I have a CVS card, my insurance is AAA, I have an annual pass at the ZOO, I have a gym membership, etc. Do I ignore this stuff or is this the type of thing they are looking for?

2008/07/21 I-751 (conditions removal) filed

2008/08/22 I-751 biometrics completed

2009/06/18 I-751 approved

2009/07/03 10-year GC received; last 0.5 done!

2009/07/23 Pras files N-400

2009/11/16 My 46TH birthday, Pras N-400 approved

2010/03/18 Pras' swear-in

Hispanic or Latino is a culture rather than a race, wife was born in Colombia, of German decent, so had to check yes to that question. For race, she is indubitably, white, whatever in the hell that means, no such race as White. But was the closest answer.
