Hello guys, I hope you can help me with this. I entered the United States with a valid K1 non- immigrant visa. Few months ago I applied for Adjustment of status with all documents included plus Medical exam from a Civil Surgeon approved by the USCIS, etc, etc. And just recently I got an RFE that says exactly:

A review of my USCIS records fail to reveal form DS-2053, DS-3024, DS-3025 or DS-3026 which are the Department of States medical form.

New form 1-693 a such, the applicant is required to submit a new form 1-693, Report of Medical Examination and Vaccination record that is properly completed and signed. The newest version of form 1-693 includes the medical information as well as the vaccination record, under the new agency guindance. Applicants two year or older are required to either have a tuberculin skin test or blood test performed.

Guys, Do I need to submit all these DS-2053, DS-3024, DS-3025 or DS-3026 along with the new for 1-693? or

just the 1-693 (We're planning to have another Civil surgeon appointment this week.)

The only DS form I have is DS-3025 which is a copy from my medical vaccination from my country. When I was filing for my AOS the Civil surgeon check my DS-3025 and transferred all my vaccination record and he gave me 2 more vaccines namely MMR and Varicella.Now in the new form they are requiring a TB or blood test.

Please Help me.

It appears to be a new (and more clearly written!!) RFE for the medical. I interpret it to mean that you need to repeat the entire medical on the I-693, complete with the TB skin test.

That may come back positive for a PI beneficiary, so be sure to bring the x-rays taken already and discuss that with the civil surgeon when you go.

Alternatively, you could pursue it through your congressperson or senator, and ask them to look into it to make sure that your POE packet was properly handled and matched up with your AOS filing. This may take some more time (won't be done in time for your CS visit next week). In our experience this route didn't completely eliminate our RFE, but it did get us 'unstuck' from that stage, and sent us to an interview where someone could explain what was wrong face to face (it wasn't at ALL what the RFE said).

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.

Ouch - sounds like your medical was misplaced with your file if they are asking for all those forms.

You may wish to contact them and see if this is just a form letter (listing all known medical forms) and if the 693 would suffice, or if they really want all those forms.

My Advice is usually based on "Worst Case Scenario" and what is written in the rules/laws/instructions. That is the way I roll... -Protect your Status - file before your I-94 expires.
