It's been a while since I've been on, but now I'm happy to be in the AOS forum instead of the K1 forum! Anyhow, my wife got her vaccinations for the visa, but there were two vaccinations that needed a second shot. Does it matter where and when she gets the shot? Can any doctor give her the shots and then can we send in the AOS package? Any help would be greatly appreciated. The whole AOS is pretty overwhelming!

In other news we went to the SSA yesterday. I try to be as kind as possible so the gal wouldn't become defensive as we requested the SSN. The problem wasn't what I had expected it to be though! We had no problem getting the card, but she said it must be issued in my wife's maiden name!! I tried and tried to explain that we wanted it in her correct name (we had the marriage cert in hand) and she finally went to ask her supervisor, but she clearly had no idea how to do anything other than just send it in in my wife's maiden name. She said to come back with the marriage cert (again) after getting the card. I hope this doesn't cause any trouble. As long as she has a number I guess that is a good start.

Any doctor can give her the shots, however you will need to take your evidence of GETTING the shots, as well as the DS-3025 to a CS to get it transcribed into an I-693.

As for SSN, you're right, you should have been able to get it in her married name but I have read of it causing a lot of issues if you try before EAD (it's not supposed to, but sometimes it happens) SO, you can always do what most people I know did, keep your SSN in your maiden name until you get the EAD, then the system has your married name in the system and there'll be no issues when you go to change it then.

I personally left my SSN in my maiden name. My AOS was processing in my married name no problem. Bank accounts are in my married name too. I still haven't changed it (in Australia at the moment) but so far no issues for the mis-match (I will change it when I get back though because I have a greencard now)

If you go the USCIS website, you can enter your zip code and receive a list of them near you. Call around as some tend to charge a lot more than others. There are a few threads on here with lists of honest CSs. You might find one near your area. I took my husband to the health dept. to get his missing vaccines as it is inexpensive if you don't have insurance yet. After we took the vaccination supplement from Brasil as well as the yellow card from the health dept. and had it all transcribed by the CS. Total cost 25 dollars. If less than a year has passed since the full medical, do not let them talk you into repeating the medical as it is costly and unnecessary. Good luck! Also, to answer your question: A CS is approved by the government to complete immigration related medical exams, etc. Your paperwork must be signed by an approved CS and not just any doctor.

No, you don't need to be good forever, you just need to be current at the time the civil surgeon signs your form in order to proceed with immigration. For your health, however, you should continue with your immuzations after the CS gives you the go-ahead.

They're authorised by USCIS to do what USCIS needs. A regular doctor can give you shots, but the CS needs to transcribe it into the I-693. The locator is here: https://egov.uscis.gov/crisgwi/go?action=offices.type&OfficeLocator.office_type=CIV

No. She only needs to be up-to-date at the time of filing for AOS. They are not permitted to delay your AOS by making you wait to get the 3rd dose. It is in your best interest to make sure she gets the final dose though when it comes time to do it. Just in case you have an interview and they ask to see it (they probably won't, but better to be sure).

She didn't HAVE an I-693 from her K1 medical, that was a DS-3025 (the vaccination part anyway, the other forms are other codes).

You don't need to have another medical done, you simply need to get the vaccinations transcribed into an I-693 by visiting a CS who will do it for a nominal fee (something like $50 or less). In order to transcribe the shots over he will need to see the DS-3025, and any further shots you've received OR you can get the shots at the CS's office to save time and make sure you're getting everything you need.

there is an AOS thread in my signature that explains this a little and gives links to threads to help.

In summury, her K1 medical is still valid, her "shots" might not be, hence the need to go to a CS, take the DS-3025 and have the shots copied over and any other shots she requires (read the I-693 instructions which explain that as a K1 you only need part 1, part 2 (vaccination table only) and part 5). This way she won't be risking an RFE because her shots aren't complete.
