That would be the copy from the K-1 medical the dr would of given you. If you didnt get a copy from the K-1 medical then you need the vaccination supliment from I-693. You take it to a civil surgeon in your area and have them fill it out and sign it. They will put it in a sealed envelope and tell you not to open it. Don't open it and just include it in your AOS package.

Shop arround at different civil surgeons. They have no set price and they will charge an arm and a leg just to fill out the vaccination portion.

( you might have to fight with the civil surgeons about that because they will try to tell you he has to have a full medical when he does not because the k-1 medical is good for 1 year. )

They would put the medical in the brown envelope that was given to POE when he entered and activated his K-1. USCIS would have the medical yes. Unless they " lost " it somewhere. That is why he would only need the vaccination supliment. If they did lose the medical then he would be sent an RFE to have a full medical done over again.

I am not sure what country you are from but you will need a copy of your DS-3025... you dont receive a copy of the medical exam but you probably were given a copy of the DS-3025, thats the record of your vaccinations! you have to take it to a Civil Surgeon and have him certify it and fill out form I-693 for you!...

A lot of people dont get copys because they dont give them out or because they didnt ask for one at the medical. If they dont have one they need the vaccination part filled out by the civil surgeon.

Juarez gives a blank DS-3025 to those there for a K-1, and refused to fill it out, even when asked repeatedly for it. They refused to give the one and only shot my fiance still needed, even when offered to be paid extra for their services.

Juarez gives a blank DS-3025 to those there for a K-1, and refused to fill it out, even when asked repeatedly for it. They refused to give the one and only shot my fiance still needed, even when offered to be paid extra for their services.

Oh that sucks.. mine filled it out and asked me to keep it because I would need it later in the process... but I read on the Visa Journey guides that if there is a way you can prove the vaccines you took the civil surgeon may not ask you to have a full exam!.. I havent experienced this part of the process tho but thats what I read on the Visa journey guides and what I have heard from other peoples experience!.. so yeah GOOD luck :D

It really does suck :angry: He explained to them he would need it later for the AOS, but they said the Consulate doesn't let them give it to people for a K-1. It all sounded really stupid silly to me. He told them they are already giving the form, so why give it blank? Why not just put the vaccinations on it? It would not take long, he was willing pay a fee for it, and he had his vaccination record with him. Nope. They would not budge at all.

He has a vaccination record, so we will try and get the civil surgeon here to fill the I-693 with that, and give him the last shot he needs, the Tdap. He still should not need another medical, but I have read the threads about how they rip people off all the time. :(

Oh well, we will make it all work out. We are approved as of today, so nothing can really take my 'happy' away right now :]

yeah I think tahts all you can do.. take all your info.. list all the civil surgeons in your area who are certified to do this and then call and see whos willing to do it..
