Hey everyone

I m going to the states very soon and we are hoping to file for AOS as soon as we can, I have been skimming through the guides because I dont want to be clueless once I get there but I have some questions concerning the medical Exam... I recently got my K1 visa so my Medical exam was in August.. we are planning to get married the lates days in Nov or first in December and then file.. now I read that if I had a medical exam before and file within one year of my overseas medical examination I was not required to take a new medical exam but I need to go to a certified civil Surgeon to certify my vaccines and fill out the form I-693.. Now I do have my DS -3025 which is the vaccination worksheet that the doctor gave me here in Venezuela.. Now Im reading it and where it says "results" he checked "Vaccine history incomplete" and also "Applicant may be elegible for blanket waiver(s) because vaccination(s) not medically appropriate (as indicated Above)"

what does that exactly mean? will I need to take a whole medical examination again? should I get more vaccines before I leave my country? I really dont want to go through the whole medical check up again so any advice or suggestion will be welcomed! thanks

No, you don't have to take another medical exam, when I went to see a doctor in Egypt he told me I'm not obliged to get all the shots the US gov. asks of me while I'm still in Egypt, but maybe the civil surgeon will want to give me more shots in the US, like hepatitis B shots or whatever.

I just filed our AOS papers last week and had to fill this out for my wife as well.

Her form DS-3025 was also marked incomplete because she had started, but had not received all of the shots required for Valicera. There were 2 shots required and she got her first one for the K1 visa and got her second shot before leaving to the USA. We found a civil surgeon who simply transcribed the results to the I-693. We provided him with the vaccination records for her valicera vaccines and he transferred them to the I-693, signed it and put them in a sealed envelope. We went in He charged us $35.00 to transcribe the results and we were on our way.

I called about several before finding one who would transcribe the results. Most of them will try to get you to take the exam a second time but its not necessary and dont let them tell you anything different. I cant remember which page but it says in the I-693 instructions that the exam you took as part of the K1 process is good for one year. I would say go down the list until you find one that will transcribe the results for a decent price.

My DS 3025 said the exact same thing (as I didn't have the chicken pox shot) and I only sent that document in with my AOS, no I-693. So far I have not received a RFE for the vaccination supplement but I guess there is techinally still time. A couple of people recently did the same thing as me and didn't receive RFEs and have had their Green Cards! However, if receiving a RFE would be a major ordeal for you and you don't want to take a risk like I did then I'd advise doing the same thing as Mike101 did! :)

what does that exactly mean?

You get a "waiver" for any shots not age appropriate or medically appropriate. It's basically automatic and not a waiver you have to apply for.

Most adults need MMR, Tdap, and Varicella (or VH written on their DS3025 because they have had chickenpox). The varicella shot is "waived" if you've had the disease and VH will indicate that.

Here's some text taken from the Technical Instructions to Civil Surgeons.

A blanket waiver is a waiver that is applied uniformly to a group of conditions and does not require a separate waiver application or fee to be filed with USCIS. In many cases, it might not be medically appropriate to administer a dose of a particular vaccine. Four “Not Medically Appropriate” categories are acceptable when determining an applicant’s eligibility for a blanket waiver.

  • Not age appropriate
  • Contraindication
  • Insufficient time interval between doses
  • Not flu season

And in the instructions of how to fill out the "results" section of the immunization form:

Applicant may be eligible for blanket waiver(s).

Completion of a vaccine series is not required to conclude the medical examination, because such a requirement would require multiple visits to a civil surgeon and could lead to unnecessary delay in the immigration process. If any of the boxes under the “Not Medically Appropriate” heading was checked, the “Applicant may be eligible for blanket waiver(s) as indicated above” box must also be checked. This box will probably always be checked because some vaccines may not be age appropriate for the applicant.

So if my wife's DS-3025 says "Incomplete" and then the checkbox about the "blanket waiver" is checked - can I just mail it along with I-485, or do we need I-693?

so did you end up getting an RFE? I'm thinking about sending it in even though it's checked incomplete as well.
