Proof of marriage is not included in the I-485 application if you are adjusting from K1 or K3.

You will have to do a complete medical. Shots are only required if you was not update to date when you entered into the US carry your shot records. But you will have to do the tb/skin test and do xray if negative and have blood drawn etc.
